The ending where Ciri becomes Empress is rather bittersweet, although it fits with the world The Witcher is set in. To get this ending, you must receive more positive influences on Ciri than negative ones. Unlike the good ending, however, you must have also brought Ciri to the Emperor.
The witcher ending is the only logical ending from the books. The empress ending just feels completely out of character. Being a witcher and free to do whatever she likes is basically everything Ciri ever wants. That (and the "prank" CDPR played on the gamers in that ending) is why I absolutely love this ending.

So, if I'm getting this right, out of the 5 decisions, if you have more positive choices than negative you should get at least the 'good' ending. Geralt didn't force her to visit Emhyr, he only answer his opinion to Ciri, that she should see her biological father, because it could be they last meet ever. And she decide for herself again. And becoming the empress is a big deal. After the game it's most powerful ruler in a world, so she can make a lot.
These are the conditions needed for the Empress ending: 1. When Ciri wants to visit the Bald Mountain during Blood on the Battlefield, you must choose "Gotta visit the Emperor first". 2. Ensure that Nilfgaard wins the war by completing: An Eye for an Eye: Help Roche save Ves. A Deadly Plot: Talk with Dijkstra and save Thaler
There are two endings in which Nilfgaard wins the war with the North. In the first ending, Ciri becomes a witcher instead of the Empress of Nilfgaard. In this "good" ending, Radovid is assassinated. Redania's cruel conquest of mages and non-humans ends. Temeria is granted some form of sovereignty as a Nilfgaardian vassal state.
As a result Ciri does not become empress as her father isn't there to hand over the throne to her. The empress ending therefore has the most requirements, you need to 1) inspire Ciri by having at least 3 out of 5 "good points" with her, 2) she needs to see the emperor, AND 3) Nilfgaard needs to rule.
